Valid Payment and Identification
You must provide a valid form of payment at the time of booking and may be required to present photo identification for verification.
Booking for Yourself
Appointments must be booked individually for yourself. If booking for someone under your care, ensure the patient’s details are accurately entered in the booking form.
Appointment Fee
You agree to pay the fee associated with your appointment, which will vary based on the type of consultation (e.g., initial or follow-up).
Non-Refundable Policy
Fees are non-refundable, except as outlined in the Cancellation Policy.
Payment Timing
Payment processing details depend on the account type established with Telgreen Medical.
Additional Costs
You are responsible for covering the cost of any medication prescribed to you, as determined by the dispensing pharmacy.
Confirmation and Adherence
Ensure you note the confirmed date and time of your appointment. Incorrect bookings are subject to the Cancellation Policy.
Patient Responsibility
It is your responsibility to ensure a quiet, distraction-free environment and a stable internet connection (if applicable) during your appointment.
More Than 24 Hours’ Notice
Appointments canceled with at least 24 hours’ notice will receive a full refund.
Less Than 24 Hours’ Notice or No-Show
Appointments canceled with less than 24 hours’ notice or missed without notification will incur the full consultation fee unless the appointment can be filled in time.
Rescheduling
If you notify us at least 12 hours in advance, rescheduling is possible with a $50 rebooking fee, subject to availability.
How to Cancel or Reschedule
Eligibility
By booking, you confirm you are 18 years or older, have a documented medical condition for at least three months, and do not meet ineligibility criteria (e.g., bipolar disorder, psychosis, or opiate treatment programs).
Information Sharing
Your health information may be shared with Telgreen Medical professionals, dispensing pharmacies, and in accordance with the Therapeutic Goods Act 1989.
Complete Medical History
You agree to disclose your full medical history, including current conditions, medications, treatments, substance use, and pregnancy status or plans.
Follow-Up Appointments
Follow-up consultations may be required to monitor your treatment progress and ensure your safety.
Treatment Acknowledgment
You acknowledge that prescribed medications may be unapproved therapeutic goods under the Therapeutic Goods Administration (TGA) and will review all provided Consumer Medicine Information. You accept the potential risks and side effects, including but not limited to:
Responsible Use
You agree to follow dosage instructions strictly, refrain from driving or engaging in hazardous activities while under the influence, and comply with all legal requirements related to your treatment.
By agreeing to Telgreen Medical’s Consent for Unapproved Medicines, you acknowledge the following:
Appointments must not be recorded under any circumstances. Telgreen Medical reserves the right to terminate any appointment if a recording is suspected and may delete unauthorized recordings.
Telgreen Medical and its healthcare providers are not liable for decisions or actions taken based on your consultation or treatment. By proceeding, you release Telgreen Medical from liability related to your participation in treatment.
By booking your appointment, you confirm that you have read, understood, and agreed to these terms and conditions.
